What is the ACCESS Program?

DocGo has joined the Center for Medicare and Medicaid Innovation 10-year ACCESS (Advancing Chronic Care with Effective, Scalable Solutions) pilot program to provide Medicare patients with a tech-enabled solution to improve their health. Through the ACCESS program, patients’ ongoing health monitoring and coaching is covered by Medicare payments under this program.

 

DocGo offers a virtual-first care for cardio-kidney-metabolic and chronic pain conditions, with remote monitoring, medication management, and optional mobile medical visits.

DocGo will report key vitals to CMS through connected devices as evidence of a patients’ improve health outcome.

Do I qualify?

Actively enrolled directly in Medicare Part B

Have diagnosed chronic conditions in the following tracks:

eCKM (Early Cardio-Kidney-Metabolic) - Qualifying Conditions: Hypertension, dyslipidemia, obesity/overweight with central adiposity, and prediabetes.

CKM (Cardio-Kidney-Metabolic) - Qualifying Conditions: Diabetes, chronic kidney disease (CKD), and atherosclerotic cardiovascular disease (ASCVD).

MSK (Musculoskeletal) - Qualifying Conditions: Chronic pain affecting bones, joints, muscles, or connective tissues.

Located in New York, New Jersey or California

What services are included in the ACCESS program?

Services that may be included as part of the ACCESS program:

Nutrition counseling
Exercise or physical activity coaching
Remote monitoring via connected devices
Medication management
Clinician messaging and consultation
Patient health education
Connection to community support services (e.g. food, housing, transportation)
Optional mobile medical visits in your home (availability limited by geography)

What is the cost?

DocGo's ACCESS program is offered at no out-of-pocket cost to Medicare Part B-insured patients through a pilot program of CMS. DocGo will bill Medicare for the service through the pilot program.
Are you a patient interested in being a patient with the ACCESS program?

Primary care physicians interested in referring your patients for ACCESS?

Primary Care Physicians (PCPs) and referring clinicians can send patient referrals directly to participating ACCESS organizations. ACCESS is built to complement your care. You can refer patients to participating organizations, receive structured clinical updates at key moments, and bill a new Co-Management Payment for your coordination — all through your existing workflows.
