INNOVATIVE HYBRID CLINICAL PLATFORM

DocGo brings the doctor’s office to the patient’s living room with deployable Mobile Health Clinical Personnel (MHCs) who are facilitate care from virtual care providers from the DocGo  Clinical Practice Group.

Our MHCs travel to patients’ homes with Bluetooth-enabled medical equipment for remote diagnostics and lab test kits to collect specimens. Advanced Practice Providers (APP) connect synchronously by telehealth during the home visit. APPs have access to the patient’s mobile diagnostics data from connected devices in the home to inform treatment plans and prescribe.

Our providers take a whole-person, preventative care approach, providing member education, SDOH assessments, care follow up and thorough documentation.
